All forms can be filled out online, scanned and emailed to Taylor Parrish. If necessary, forms can also be hand-delivered to the Office of Graduate Programs (on the second floor of the Hardin Administration Building) or to the office of the Thesis Coordinator (Room 129 of the Phillips Education Building); however, email is preferred.

The semester before you enroll in thesis hours, fill out the following form:
