The Intent to Graduate form is available online. Submit the Intent to Graduate online form at least one year prior to expected graduation.  

The Application for Comprehensive Exam/Thesis Defense forms are also available online.  The Comps/Thesis Defense form is in MS Word format; open and save to your computer.  The Comps/Thesis Application should be submitted directly to the Graduate School (electronically as an attachment to an email, or print and send the paper copy). For information on a specific commencement ceremony, please click on one of the three below.
