Immunization Requirements for Graduate Students

Immunization Requirements

To ensure the health and safety of our campus, immunization against communicable diseases is extremely important. Vaccination against meningococcal meningitis, measles, and tetanus is required of all new, transfer, readmitted, graduate and special students enrolling following a break of at least one long semester at ACU.

  • The student must have received at least two vaccinations for measles.
  • The tetanus vaccination must have been given within the last ten (10) years.
  • The initial meningococcal vaccine or booster must have been administered during the five-year period preceding and least ten (10) days prior to the first day of the first semester in which the student initially enrolls at an institution.

Exemptions

Students who meet any of the following conditions may be exempt from these requirements: 

  • Students who are enrolled only in online classes
  • Students who are at least 30 years old
  • Students who graduated from ACU and will immediately begin graduate work without a break of one long semester (Example: A student who completed his or her undergraduate work in May 2012 at ACU and will begin graduate work at ACU in August 2012)

Immunization Form

Before enrolling in classes, all new, transfer, readmitted, graduate and special students enrolling following a break of at least one long semester at ACU must submit a completed copy of the Graduate Student Immunization Form that has been signed by a health care professional or is accompanied by copies of your official immunization records.

Acceptable Records

Acceptable records demonstrating your immunizations may be obtained from any of the following (no other documentation will be accepted):

  • High school or previous college or university records – These may contain some, but not all of your immunization information. They must have an official stamp or signature. Your immunization records do not transfer automatically. You must request a copy.
  • Personal shot records – Must be verified by a doctor’s stamp or signature or by a clinic or health department stamp.
  • Local health department
  • Military records or WHO (World Health Organization) documents

Be certain that your name, date of birth and ID number appear on each sheet and that all forms are submitted together. The records must include the dates of vaccine administration – including the month, day and year. All records must be in English. Please keep a copy for your records.
