If you are entering Abilene Christian University for the first time, you will need to complete the Student Immunization form before July 15.

Print this form and then have it completed and signed by a health care professional, or complete this form and submit it along with an official photocopy of your records. 

Submit your completed form to Student Immunization Form, ACU Box 29000, Abilene, TX  79699-900 or fax it to 325-674-2130.  If you have any questions, call our office 325-674-2625.
