Hazard Communication Training

In accordance with the Employee Training requirements (see section IV. F. in Employee Training Policy), supervisors will ensure that all employees receive initial Hazard Communication training and complete the computer-based Hazard Communication training. Training should be accomplished during the office/department orientation and recommend documentation be maintained in the workplace.

When completing the online computer-based Hazard Communication training, please fill in the following blanks completely:

2. Last name

3. Trainee SSN or Employee No: (we recommend using your Banner number) PLEASE DO NOT ENTER YOUR SOCIAL SECURITY NUMBER)

4. Your score will be mailed to: Please insert YOUR email address and retain a copy of the email for your supervisor.
