A new copy of your Social Security Card can be obtained by visiting the Social Security Administration Office at:

Please be prepared to present proof of identification (driver's license, etc.) in order to request a new Social Security Card.  A stamped and initialed print-out will be given to you.  Human Resources will utilize that stamped and initialed form as your Social Security Card.
