Schedule Appointment - New Client
If you are student, staff, faculty or dependent of ACU and interested in receiving our counseling services and want to make an appointment, please print and complete the "Intake Form". Bring this form with you to our office, located at 1750 Campus Center Road, McKinzie Hall, Lower Level, Room 3D.
An appointment will be made with a therapist within 48 to 72 hours upon receiving your intake form.
