Small Group Application

Before you apply to begin a Small Group Chapel, please read the Small Group Guidelines.

The form below is an application for being part of the Small Group Chapel programs. The form must be filled out completely and submitted at least one week before the Chapel assembles each semester.

After the application is submitted, a confirmation email will be sent with further instructions to the contact person and advisor.
