Complete the Disclosure Statement and Residency Affidavit (DSRA)
The Disclosure Statement and Residency Affidavit is a required document for ACU. The purpose of this form is to verify financial aid eligibility.
Please download the application. Then fill out the word document of the DSRA and either mail or fax it into ACU's Student Financial Services Office:
