Student Immunization Form

All students are required to submit a Student Immunization Form before they may begin classes at ACU. 

In addition to traditional vaccinations, such as the MMR (Measles, Mumps and Rubella), the state of Texas requires all new college students, who plan to move into on-campus housing regardless of their state of residency, to be immunized against bacterial meningitis at least 10 days prior to moving into on-campus housing
