Faculty Testing Accommodation Checklist

If you have a student with a documented disability who uses accommodations through the Alpha Scholars Program, please print, complete and sign this test checklist. Attach the signed form to each test you send to them Alpha. It is imperative that Alpha receives this information directly from the professor for each test, especially during finals.
