2007 Big Purple Registration Form

Please complete as much of the following form as possible. Many of the fields are marked "required" - you must provide a response for these questions or the form will not be submitted. Please provide accurate information.
Freshmen and transfer students may not have been assigned their ACU address, phone number or banner number at this time. You may complete this information when you register during Welcome Week.
There is a $50.00 per semester rental fee for each instrument. Percussion and Color Guard members will also be charged this equipment fee.
Every member of the Big Purple Marching Band will need a YBP Polo Shirt, a 2007 YBP t-shirt, black marching shoes, and white gloves. (Percussionists and Color Guard will not need gloves. Guard shoes will be ordered in place of marching shoes for all guard members.) NOTE: If new members have existing black shoes and white gloves in good condition, they may be used in place of ordering new ones.
The Big Purple baseball cap is optional. However, this is the only hat that can be worn with your uniform in the stands on game day.
